If I don't let off the gas it will not shift out of 1st gear until about 4k RPM. Letting off the gas will allow it to shift sooner. It shifts out of second fine. However... this is what happens when I use the gear shifter to hold it in second gear.

Throw a can of BG or Lucas in it and see what you get. What's the fluid look like?

ok it sounds like its skipping 2nd and going into 3rd which is a common problem if it has a band problem or a reaction shell issue have you check manuel 2nd gear
